A breast reduction and breast lift is often combined with a breast augmentation in order to provide more support to the breasts. If the same breast size is desired then the removed sagging breast tissue is replaced by a silicone or Saline filled implant. If an increase in breast size is desired then the excised breast tissue is replaced by a larger implant. If a breast reduction is requested then the removal of the breast tissue is more generous and it is replaced by a smaller implant. If a significant breast reduction is desired then maximum removal of the sagging breast tissue is accomplished without an implant. The procedures can all be done at the same time in most cases. General anesthesia and tumescent anesthesia are usually employed for a better patient experience with less risks. A mastopexy ( Breast lift) is usually done through a lollipop or keyhole incision (A circle around the Areola of the nipples) and a vertical incision from the bottom of the areola which extends to approximately 1 inch above the inframammary folds (The crease under your breasts). The Wise pattern or anchor incision is rarely necessary and it involves an additional incision ( wound) along the inframammary folds for the removal of excess breast tissue and excess skin which is indicated for the very large breasts. A superior medial Pedicle consists of preserving the major vascular supply to the nipple intact and rotating the nipple upwards to a more aesthetically pleasing position.
